Physical Exams

School physicals needed by grade for Illinois students (click the link to download the form):

Preschool: Physical and dental examinations are required.
Kindergarten: Physical, dental, and vision examinations are required (including lead and diabetic screening).
Second: Dental examination is required.
Fifth: No physical is required any more, but students will still need to meet required Hepatitis B guidelines for entrance to 5th grade. Letters will be sent home to inform families if immunizations are needed.
Sixth: Physical and dental examinations are required.

Physical, dental and eye exam forms are to be turned in by the first day of student attendance.

All students playing sports need a physical. The sixth grade physical will count for sixth grade. Fifth, seventh, and eighth graders should use the physical sports' form (download both sides).
